Testing Algorithm

This test is designed to hold the sample and delay FISH testing while preliminary morphologic assessment or flow cytometry testing is in process.

HOLD policy: Upon sample receipt, the specimen will be processed and held in the laboratory. FISH testing will not be performed unless the client contacts the laboratory and indicates the FISH testing is desired. The client must contact the Cytogenetics Laboratory at 507-266-0790 by 4 p.m. (central time) no later than 4 business days (96 hours) after the specimen was collected to notify the laboratory of the FISH testing desired. If no notification is received by this time, the HOLDF orders that do not have a FISH test indicated will be processed as "cancelled" and a processing fee will be charged. Weekend communication can be deferred until Monday.

Specimen Type

Varies

Provide a reason for referral with each specimen and bone marrow pathology report (if available). The laboratory will not reject testing if this information is not provided, but appropriate testing and interpretation may be compromised or delayed.

This test cannot be utilized when ordering PCPDF / Plasma Cell Proliferative Disorder (PCPD), FISH.

Forms: If not ordering electronically, complete, print, and send a Hematopathology/Cytogenetics Test Request Form (T726) with the specimen (http://www.mayomedicallaboratories.com/it-mmfiles/hematopathology-request-form.pdf)

Advise Express Mail or equivalent if not on courier service.

Submit only 1 of the following specimens:

Specimen Type: Whole blood

Container/Tube: Green top (sodium heparin)

Specimen Volume: 7-10 mL

Collection Instructions:

1. Invert several times to mix blood.

2. Other anticoagulants are not recommended and are harmful to the viability of the cells.

Specimen Type: Bone marrow

Container/Tube: Green top (sodium heparin)

Specimen Volume: 2-3 mL

Collection Instructions:

1. It is preferable to send the first aspirate from the bone marrow collection.

2. Invert several times to mix bone marrow.

3. Other anticoagulants are not recommended and are harmful to the viability of the cells.

Specimen Minimum Volume

Blood: 2 mL; Bone Marrow: 1 mL

Clinical Information

FISH analysis using gene-specific probes is a useful methodology to detect common, recurrent chromosome abnormalities for most hematologic malignancies. Based on morphologic review of the bone marrow or peripheral blood specimen by a hematopathologist, a determination of additional appropriate testing can be made. If the specimen does not show evidence of malignancy, FISH analysis may not be necessary. Depending on the diagnosis, conventional chromosome analysis may also be more informative.

Cautions

This test does not apply to chromosome analysis. If chromosome analysis is to be held, then HOLDC / Hematologic Disorders, Chromosome Hold, Bone Marrow or Peripheral Blood must be ordered.

This test cannot be utilized when ordering PCPDF / Plasma Cell Proliferative Disorder (PCPD), FISH.
